A fetal echocardiogram is primarily performed between 18 to 22 weeks of gestation. This timing is crucial as it allows for adequate visualization of the fetal heart structures during a period when they are fully developed but still within a timeframe where potential issues can be detected and managed effectively. By this gestational age, significant cardiac anatomy is established, and any abnormalities can be assessed with sufficient clarity.

Earlier in the pregnancy, specifically before 15 weeks of gestation, the fetal heart is still developing, and the structures may not be easily visualized, making it challenging to accurately assess for congenital heart defects. Similarly, while fetal echocardiograms can be performed after 30 weeks, this is generally not the standard practice for routine evaluations, as by that time the heart structure evaluation is usually already done during the earlier window. Conducting the echocardiogram anytime after the first trimester could miss the ideal window for comprehensive evaluation, as well. Therefore, the 18 to 22 weeks gestation period is the optimal timeframe for this important assessment.
